USAG REGISTRATION, PAYMENT/PAYMENT FORM DUE 11/5/2024

**Our meet fills up quickly and registration will close early when full.

Registration will not be final or guaranteed until all of the above is submitted.**

REFUND/CHANGES POLICY:

* There will be NO REFUNDS after 12/1/2024

* Level changes will be honored if space is available.

ENTRY FEES:

USAG Levels 6-10/Open:      $150.00 per Athlete

USAG Levels 3-5:                   $125.00 per Athlete

USAG Xcel:                              $125.00 per Athlete

Team Fee:                               FREE

ENTRIES AFTER 11/5/2024 WILL INCUR A $10 LATE FEE PER GYMNAST

REGISTRATION IS EASY! 

1) Go to USAG Meet Reservation System to enter athletes and coaches with verified memberships. 

2) Print and fill out the Womens Payment Form.  Boys Payment Form to come.

3) Print and fill out the Leotard Order Form. All gymnasts will receive a Winter Classic Leotard as this year's meet gift.

4) Mail your check and Payment Form to:

    Chow's Booster Club, P.O. Box 65181, West Des Moines, IA 50265.
